财务责任通知

费用的支付/支付承诺

I understand that when I register for any class at the 道成肉身的大学 (UIW) or receive any service from UIW I accept full responsibility to pay all tuition, fees, 由于我的注册和/或接受服务而评估的罚款和其他相关费用. I understand and agree that my tuition bill is due on/by the first day of the semester OR I must be on a payment plan. I further understand and agree that my registration and acceptance of these terms constitutes a promissory note agreement. 我保证支付所有的学费, fees, 在公布或指定的到期日之前支付其他相关费用. I understand and agree that if I drop or withdraw from some or all of the classes for which I register, I will be responsible for paying all or a portion of tuition and fees in accordance with the published tuition refund schedule per Academic Calendar. I have read the terms and conditions of the published tuition refund schedule and understand those terms are incorporated herein by reference. I further understand that my failure to attend class or receive a bill does not absolve me of my financial responsibility as described above.

Method of Billing

我了解到UIW使用电子计费(e-bill)作为其官方计费方式, and therefore I am responsible for viewing and paying my student account e-bill by the scheduled due date. I further understand that failure to review my e-bill does not constitute a valid reason for not paying my bill on time. 电子帐单及缴费计划资料可于 Cardinal Apps in my ePayment Center.

学生健康保险

I understand and agree that as a Domestic full-time main campus student (12+ hours for undergraduate*, 6+ hours for graduate* and 6+ hours for professional) and as an International student enrolled in 1+ hours (eligibility is based on admission status) I am required to participate in the student accident and sickness insurance plan. ONLY 国内学生可凭保险证明免保. I understand that the insurance charge is automatically assessed and must be waived online through the Wellfleet学生保险 (截止日期请参考校历).

*does not include those registered in the 专业研究学院 (in-person/Virtual) or main campus graduate students enrolled in 8-week terms (G1 or G2)

Billing Errors

我理解行政管理, clerical, or technical billing errors do not absolve me of my financial responsibility to pay the correct amount of tuition, 由于我在UIW注册而评估的费用和其他相关财务义务.

退回付款/未付款协议

I understand and agree that a $25 insufficient fund (NSF) charge will be assessed to my student account for any payment (check, ACH) returned unpaid. I understand multiple returned payments and/or failure to comply with the terms of any payment plan or agreement I sign with UIW will result in the suspension of my ability to submit future payments via ACH or checks and/or future registration with UIW.  

Withdrawal

隐私权与责任

I understand the Family Educational Rights and Privacy Act (FERPA) prohibits UIW from releasing any information from my financial and/or education record without my written permission. Therefore, I understand and agree that if I want UIW to share information from my financial and/or education record with someone else, 我必须按照下列程序提供书面许可 FERPA，可在注册办公室网页-隐私法信息中找到. I further understand that I may revoke my permission at any time as instructed in the same procedure.

IRS Form 1098-T

拖欠帐户/集合

  • Financial Hold - I understand and agree that if I fail to pay my student account bill or any monies due and owing UIW by the scheduled due date, UIW将冻结我的学生账户, 阻止我注册以后的课程, 请求记录, 领取毕业证书或参加任何毕业活动(毕业典礼).
  • Late Payment Charge - I understand and agree that if I fail to pay my student account bill (due on the first day of the respective semester) OR sign up for a payment plan by the scheduled due date, uw将在11月5日之前收取250美元的滞纳金th (秋季学期)和4月5日th (spring semester).
  • 代收代理费 - I understand and accept that if I fail to pay my student account bill or any monies due and owing UIW by the scheduled due date, 未能作出可接受的付款安排，使我的帐户保持畅通, uw可能会将我的欠款转介给催收公司. 本人进一步明白，本人有责任支付托收代理费(最高可达33英镑).我拖欠账户余额的33%, 连同所有成本和费用, 包括合理的律师费, 必须用来收回我的欠款. Finally, I understand that my delinquent account will be reported to one or more of the national credit bureaus.

Communication

  • 沟通方式 - I understand and agree that UIW uses e-mail as an official method of communication with me, and that therefore I am responsible for reading the e-mails I receive from UIW on a timely basis.
  • Contact - I authorize UIW and its agents and contractors to contact me at my current and any future cellular phone number(s), 关于拖欠学生账户/贷款的电子邮件地址或无线设备, 我欠uw的任何其他债务, 或向澳门大学索取一般资料. 本人授权uw及其代理和承包商使用自动电话拨号设备, 人工的或预先录制的声音或文字信息, 私人电话和电子邮件, 在他们努力联系我的时候. Furthermore, I understand that I may withdraw my consent to call my cellular phone by submitting my request in writing to the UIW business office or in writing to the applicable contractor or agent contacting me on behalf of UIW.
  • 更新联系人信息 - I understand and agree that I am responsible for keeping UIW records up to date with my current physical addresses, email addresses, and phone numbers by following the procedure at UIW website for updating student address/email address/phone number. I understand and agree to provide UIW with updated contact information for purposes of continued communication regarding any amounts that remain due and owing to UIW.

Entire Agreement

本协议取代所有先前的谅解, representations, 学生与UIW之间的协商和通信, 构成双方就所述事项达成的完整协议, 不受任何交易过程或履行过程的改变或影响. 如果我签署修改，uw可能会修改此协议. Any modification is specifically limited to those policies and/or terms addressed in the modification.
